    If it is necessary for your child to receive medication during the school day, please have the physician complete the Medication Administration form. No medication will be administered unless the form is completed and signed by the physician and parent. Students are not permitted to carry or self-administer any medication at school, even over the counter medication. If the child's physician uses a medication order/form specific to his/her practice, please ensure a parent also signs that form. 

    No medication will be accepted by the health office if not accompanied by proper documentation. Medications prescribed and filled by a pharmacy must be in the original container with the student's name and other information clearly legible. Medication information must match the physician's order. Over the counter medications must be in original packaging and cannot be expired.

    Medication administration forms must be renewed for each school year.
